Predictive-modeling tool supports hospice patients and their families during their last days
The number one goal of hospice care is to enhance the patient’s quality of life by managing their symptoms and keeping them as comfortable as possible.
The last days and hours of your loved one’s life can feel like a roller coaster. Uncertainty, grief, and worry can be overwhelming for them and you.
To help nursing provide more personalized care during those last days and hours, Knute Nelson adopted a new predictive modeling tool.
How predictive modeling helps patients and families
The tool analyzes over 900 data points from nursing and social work visits to predict patients who are most likely to pass away within 7-10 days. This information, combined with the expertise of the team, helps staff provide the right care at the right time.
“This new technology ensures that we're offering the most appropriate care when its needed,” said KaLee Mohrman, Director of Hospice. "Our most recent data has shown that our team provides an average of 10.47 visits in the last 7 days of a patient’s life – this is nearly double the national average of 5.4 visits. This technology enables us to enhance the quality of care of patients and their loved ones receive at the end of life.”
During their loved one's last days, this insight can help families relieve some uncertainty and fear. It also helps hospice staff offer the emotional and spiritual support they may need when they need it most.
