Hospice Program Recognized For Quality of Care

Our hospice program was honored with the Hospice Quality Excellence Award from Homecare Homebase, as part of their Client Success Awards. The award is given to a hospice provider and Homecare Homebase client who demonstrates their commitment to superior quality of care through their assessment process, as measured by the Centers for Medicare and Medicaid Services.

The assessment, Hospice Item Set, comprises care process measures, including asking patients about their beliefs and values, asking patients about their treatment preferences, and checking on patients to determine if they are in pain or experiencing shortness of breath.
